Your Recovery Path

A Plan That Moves With You

Day 0

Discharge Day

Our care coordinator reviews your discharge summary with the hospital team, confirms medication lists, and arranges transport home.

Day 1–3

First 72 Hours

A nurse visits within hours of arrival to set up the home care space, check vitals, and walk your family through wound care.

Week 1–2

Stabilization

Scheduled follow-ups track healing, adjust medication dosages with your physician, and catch early warning signs.

Week 3+

Independent Recovery

As strength returns, we taper visit frequency, hand off self-care routines, and stay one call away.

Why It Matters

Readmissions Rarely Start in the Hospital

Most post-discharge complications trace back to a missed medication dose, a delayed follow-up, or a wound that wasn’t checked in time. A structured transition closes those gaps before they widen.

SIRIS Healthcare’s discharge-to-home program keeps a single care coordinator across the entire recovery window, so your family is never re-explaining the case to someone new.

Common Questions

Before You Bring Them Home

How soon after discharge does care begin?+

Our team is briefed while you're still in the hospital. In most cases, a nurse reaches your home within 48 hours of discharge, and sooner for critical cases.

Who decides how long care continues?+

Your treating physician and our care coordinator jointly review progress every week and adjust the plan based on recovery, not a fixed contract length.

Can you coordinate directly with my hospital?+

Yes. We request discharge summaries and treatment notes directly from the hospital's care team so instructions are never relayed secondhand.

What if a complication comes up outside visit hours?+

Our nurse hotline is staffed around the clock. We triage over the phone and can dispatch a home visit or coordinate ER transfer if needed.

Coming Home Soon?

Talk to a care coordinator before discharge day so the plan is ready the moment you walk through the door.
